Overview
Band 7 Specialist Mental Health Practitioner with responsibility for transitions across CAMHS mental health services. The role collaborates with pathway Clinical Leads to coordinate transitions from CAMHS to Adult Mental Health Services, or step down to Primary Care in line with the CYP Transitions policy. Transitional oversight includes Community CAMHS, CAMHS Inpatients, Intensive Outreach Team, CAMHS Crisis, CYP Community Eating Disorder Service, Mental Health Support Teams and the CYP ADHD/ASD Team. The role carries a 60% caseload, providing care coordination and evidence-based intervention based on children & young people\'s individual needs.
Key Responsibilities
* Work with the Clinical Lead to support CYPMH Transitions protocols, offering consultation, training and guidance to clinical staff on Transition protocols; raise the profile of young adults, SEND requirements and carers\' access to services while developing best practice.
* Promote transition planning: meeting with the service user at least six months before transition (or at least one month before transition if age is near threshold) and ensure the meeting includes the young person, the sending lead professional, a transition contact where applicable, and the young person\'s parent(s)/carer(s) where appropriate.
* Agree transitions plans collaboratively among the sending and receiving services, the Young Person, and where appropriate the Young Person\'s parent or carer; include jointly agreed personal transition goals with signposting to wider community services.
* Support and develop robust clinical protocols with Clinical Leads, providing consultation, supervision, training and ensuring best practice to families and carers.
* Contribute to policy development and service improvement to enable seamless services across organisational boundaries and partners.
* Collaborate across the health community with Service Managers, Clinical Leads and NHFT/NCC partners to deliver high-quality care.
* Foster a strong customer focus and user engagement to gather timely feedback that supports rapid innovation and improved outcomes.
* Challenge complacency and identify activities not yielding demonstrable improvements in patient outcomes, promoting staff and team ownership of change.
* Respond to complaints in accordance with Trust policy and support incident reporting and learning.
* Adhere to infection control precautions and safeguarding requirements; raise matters with the named leads per policy.
* Represent the service at local and national events as required and undertake other duties commensurate with the band.

Qualifications
Essential
* Registration with an appropriate governing body (e.g., NMC, BACP, BABCP, HCPC).
Desirable
* Continuing supervision in line with registering body requirements for continuing professional development.
Experience
Essential
* Knowledge of a range of mental illnesses in children and young people, and ability to work with NICE guidance and evidence-based practice.
* Demonstrable knowledge and skills in evidence-based interventions.
Desirable
* Post-qualification experience of working in community settings.
* Good knowledge of local children\'s services.
