GP Care Navigator

Closing date: 23 June 2026.

We are looking to recruit a GP Care Navigator to join us at North House Surgery. Your role will be to receive, assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective manner. You will support the practice team and project a positive and friendly image to patients and visitors, either in person or via the telephone.


Job Summary

Responsible for a wide range of reception and administrative duties and providing general support to the practice team. Duties include greeting and directing patients, patient registration, booking appointments, processing information and assisting patients as required. Acts as the central point of contact for patients and liaises with the clinical team and external agencies.


Primary Responsibilities

* Cover reception on a rotational basis during surgery opening hours.
* Greet and direct patients and visitors politely and courteously.
* Make accurate appointments for doctors, nurses and other team members using the computer system.
* Handle general enquiries and explain procedures.
* Use judgment and communication skills to see patients in urgent consultation logically and non-disruptively when no prior appointment exists.
* Assist with administrative tasks as required.
* Answer the telephone promptly and politely, divert calls and take messages ensuring accuracy and appropriate delivery.
* Process repeat prescription requests promptly and accurately.
* Enter requests for home visits onto the computer system, recording all details and referring to doctor when necessary.
* Advise patients of relevant charges for private services, accept payment and issue receipts.
* Enter patient information onto the computer as required.
* Undertake other duties as requested by partners, practice manager or reception supervisor.
* Process personal, telephone and e‑requests for appointments.
* Receive, assist and signpost patients and visitors to the correct service or professional.
* Process incoming and outgoing mail.
* Initiate contact and respond to requests from patients, team members and external agencies.
* Input and process data within the clinical system (referrals, read coding, letters, registrations).
* Photocopy, file, store and scan documentation as required.
* Process requests for medication and home visits efficiently.
* Manage all queries, including administrative queries, efficiently.
* Carry out system searches and maintain a clean, tidy working area.
* Monitor and maintain reception area and notice boards.
* Support clinical staff with general tasks as requested.
* Complete opening and closing procedures for premises.


Person Specification


Qualifications

* Educated to GCSE level or equivalent.
* GCSE Mathematics & English (C or above).

* Excellent written and oral communication skills.

* Competence in the use of Office and Outlook.
* Effective time management and planning.
* Ability to work as a team member and autonomously.
* Good interpersonal skills.

* Experience of working with the general public.
* Experience of administrative or receptionist duties.
* Experience of working in a healthcare setting.

North House Surgery is a GMS practice located in Crook with a list size of 13,500. Our practice features a GP Partners team, salaried GPs, GP registrars, ANPs, nurses, health care assistants, a phlebotomist, pharmacists, pharmacy technicians, social prescribers and wellbeing coach. We have a strong focus on education, run regular in‑house teaching sessions and maintain a Good rating from CQC inspections.
