Job Details
2 x Post – permanent and full time (37 hours per week). Part time applicants will be considered.
Band 4 / Band 5 depending on experience.
Responsibilities
* Travel within Perth & Kinross Health and Social Care Partnerships.
* Work in GP practices, pharmacy hubs, care homes, community hospitals and patient homes with an emphasis on high quality patient centred pharmaceutical care.
* Execute clinical pharmacy activities within a multidisciplinary team.
Qualifications
* Registered with the General Pharmaceutical Council of Great Britain (GPhC) or in application process.
* Experience of primary care desirable.
* A personal and professional philosophy that puts patients at the centre of service activity.
* The ability to work independently and as part of a team.
* Effective communication and interpersonal skills.
* The ability to establish and maintain relationships in a multidisciplinary team.
* The ability to adapt to change and meet tight deadlines.
