Overview
Lincolnshire Community Health Service (LCHS) have an opportunity for Registered Nurses to join their Grantham Community Nursing Team. We have vacancies for 1 full-time permanent position and 2 part-time positions.
Responsibilities
* Work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings.
* Assess care needs and plan, deliver and evaluate nursing care using evidence-based practice.
* Provide interventions including wound management, continence care, palliative care, medication administration and long-term condition management.
* Collaborate with the MDT (Social Workers, GPs and Allied Health Professionals) to ensure coordinated patient care and positive outcomes.
