Urgent Response is a fast-paced, interdisciplinary community team aimed at preventing unnecessary hospital admissions and facilitating timely discharges. This integrated health and social care team visits patients at home and completes assessments in ED, bridging hospital and community care. Patients are over 18 with various health and social needs.
The role involves day-to-day management of a designated caseload requiring nursing, rehabilitation, or care management interventions. The nurse will organise and prioritise treatment programmes, with cases reviewed periodically by senior staff.
The nurse will work within the community and in University Hospital Southampton ED as a lone practitioner, managing risk situations with telephone support. Support and advice from experienced colleagues should be sought when cases are outside the nurse’s scope of responsibility, knowledge, or skills.
The nurse must obtain informed consent for treatment and share information in accordance with Trust and Local Authority procedures, including consent for clients unable to give informed consent.
Assessments include:
* Generic health and social care assessments using the Single Assessment Process (SAP)
* Comprehensive nursing assessments for clients with complex or chronic needs, including tissue viability, dysphagia, nutritional risk, continence, care packages, equipment fitting, moving and handling risks, and medication management.
