Job Summary
An exciting and challenging opportunity has arisen for a permanent full-time Band 6 Occupational Therapist within the Integrated Independence Team (Rapid Response Team). We are looking for an innovative, enthusiastic and experienced Occupational Therapist to be part of the Emergency department and community rapid response service within the Integrated Independence Team. This will be offered as permanent and full-time for an existing Band 6 therapist or experienced Band 5 therapist.
Working Hours: 37.5 hours per week (part time, flexible working or compressed hours will be considered).
Main duties of the job
Provide a rapid response, senior Occupational Therapy assessment and treatment of complex patients within Emergency department, short stay unit and patients referred from the community in order to prevent admission to hospital and promote early discharge. Work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.

About the Integrated Independence Team
The Integrated Independence Team is an integrated health and social care service provided jointly by Homerton University Hospital NHS Foundation Trust and London Borough of Hackney. The team places the user at the heart of the care planning process, aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.
The team provides rapid response, intermediate care therapy and reablement services and consists of Occupational Therapists, Physiotherapists, Psychotherapists, Nurses, Speech and Language Therapists, Consultant Geriatricians, Social Workers, OT Technician, Care Support Coordinators, Care Support Workers, Therapy Assistants and Administration staff. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.
Qualifications, Skills and Experience
* Education and qualifications:
o Degree / diploma in OT recognised by RCOT
o HCPC registration
* Experience:
o Post graduate experience in general physical field of OT
o Experience of working in acute hospital setting
o Experience in contributing to service development projects
o Experience of working in a community setting
* Essential skills and abilities:
o Ability to assess clients with complex physical disabilities
o Able to manage a clinical caseload and prioritise as required
o Senior level of clinical skills
o Demonstrates effective communication skills with a variety of agencies to optimise patient/client management
o Sound IT skills, e.g. Word processing and e-mail
o Able to problem solve, plan and manage time effectively
o Awareness of own competency level and how to seek appropriate help
o Ability to work effectively within a multi-disciplinary team
o Ability to reflect and adapt practice to suit individual's needs
o Demonstration of effective decision-making skills
o Clear, concise verbal and written reporting skills
