We are looking to recruit a dedicated and experienced Paramedic to join our friendly and supportive team at Chiltern House Medical Centre on a part-time basis. This flexible role offers an excellent opportunity for a Paramedic seeking a better work-life balance while continuing to provide high-quality, patient-centred care.

The successful candidate will work as part of our multidisciplinary team to assess, diagnose, and treat patients presenting with a wide range of minor illnesses and urgent care needs. You will play a key role in improving access to clinical services, supporting chronic disease management, and contributing to the overall health and wellbeing of our local patient population.


Main duties of the job

The Paramedic will act as a specialist clinical resource, working autonomously within the community to assess, diagnose, treat, refer, or signpost patients presenting with undifferentiated or undiagnosed conditions related to minor illness or injury. Collaborating closely with the wider multidisciplinary team, the role involves delivering holistic, patient-centred care, supporting self-care and informed decision-making. The post holder will also be required to carry out home visits and must have access to a vehicle, with reasonable travel expenses reimbursed in line with the organisations policy.

Job responsibilities

Clinical:

* Undertake homevisits as assessed and when appropriate
* Assessment andtriage of acute presentations including telephone triage
* Contributing towardsthe running a minor illness/ injury walk-in service
* Assisting in thetraining of frontline staff
* Make directreferrals to primary, secondary and social services within locally
* agreed pathways,guidance and protocols
* ECG and ABPMinterpretation
* Assisting with otherad hoc clinical tasks
* Ensure complete andaccurate documentation of every patient contact

Professional:

* Participate in anannual individual performance review, taking responsibility for maintaining arecord of own personal and professional development
* Undertakeprofessional development as required by the role to ensure thatup-to-dateevidence-based knowledge and competence in all aspects of the role is maintained
* Monitor theeffectiveness of your clinical practice through quality andassurance strategiessuch as the use of audit and peer review
* Adhere to practicepolicies, procedures and guidelines
* Diagnosing andtreating patients presenting with minor illness
* Triage and treatpatients wishing to see a health care professional, making anynecessary referrals toother members of the primary health care team
* Patients withproblems needing referral to secondary care should be discussed with the registered GPbefore making such referral.
* Ensure clinicalpractice is safe and effective and remains within boundaries ofcompetence, and toacknowledge limitations.
* Advise patients ongeneral health care and minor ailments, with referral to other members of the primaryand secondary health care team as necessary
* Works from thesurgery and within communities as an autonomous practitioner caring for patientsand families.
* Works as anautonomous practitioner, in accordance with the Health Professions Council. Ensure thatpersonal and professional clinical standards are maintained.
* To undertakeassessment for patients within the community and those attending the surgery, usingdiagnostic skills and initiation of investigations where appropriate.
* Prescribe/issuemedications as appropriate following policy, patient group directives and localpathways.
* To formally andinformally impart knowledge and skills to colleagues promoting peer review and bestpractice within the work environment.
* To communicate atall levels within the team ensuring an effective service is delivered.
* To maintainaccurate, contemporaneous healthcare records appropriate to the consultation.
* Ensureevidenced-based care is delivered at the highest standards ensuring delivery of highquality patient care.
* Works with localpolicies and procedures.
* Enhance ownperformance through Continuous Professional Development, imparting ownknowledge and behaviours to meet the needs of the service.
* To achieve anddemonstrate agreed standards of personal and professional development in orderto meet the needs of the service.
* To participate inthe audit process, evaluation and implementing plans and practice change inorder to meet patient need.
* Contributepositively to the effectiveness and efficiency of the team and work colleagues.
* Pathological specimensand investigatory procedures
* Undertake thecollection of pathological specimens including intravenous blood samples, swabs etc.Perform investigatory procedures requested by the GPs


Person Specification


Qualifications

* HCPC Registration: Must be a registered Paramedic with the Health and Care Professions Council (HCPC).
* Recognised Paramedic Qualification:
* BSc (Hons) in Paramedic Science, or
* DipHE in Paramedic Practice (for those qualified before degree-level entry became standard).
* Advanced Clinical Assessment Skills: Demonstrated ability to assess, diagnose, and manage patients with minor illness or injury.
* Experience in a Primary Care, Community, or Urgent Care Setting: Previous experience delivering autonomous care outside of the ambulance service is often desirable.
* Excellent Communication and Interpersonal Skills: Ability to work collaboratively within a multidisciplinary team and communicate effectively with patients.
* Full UK Driving Licence: Required for home visits and community work.
* Evidence of Continuing Professional Development (CPD): Up-to-date CPD portfolio relevant to scope of practice.
