Substance Use Nurse - Western Bay Clinical Service
Substance Use Nurses will deliver services in line with Welsh Government Treatment Frameworks, NICE guidance (National Institute for Health and Clinical Excellence), and other relevant professional standards and legislation. Kaleidoscope staff are expected to follow Operational Policies and Standard Operating Procedures (SOPs), ensuring that the care and treatment provided is safe, undertaken to a high standard and informed by up-to-date, evidence-based practice.
The post holder should enjoy face-to-face contact with people who have recognised that their use of substances has become problematic. They will have excellent communication skills, be able to work as part of a multidisciplinary team, across agency cultures including those within statutory, private and voluntary sector partnerships. The post holder must be flexible and efficient within small locality teams, to ensure services are delivered equitably and with the service user at the heart of delivery.
Main duties of the job
* Deliver onsite dispensing and supervision of substitute opiate medication.
* Follow Controlled Drug Policies and Procedures.
* Provide advice, information and guidance on the management of substance use to clients engaged with services.
* Undertake comprehensive clinical assessments.
* Agree treatment plans with clients, and review them regularly.
* Undertake drug screening (urine and oral).
* Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols.
* Deliver clinical interventions, including HCV testing, HBV immunisation and naloxone training/administration where appropriate and in partnership with Local Health Board and PHW colleagues.
* Deliver subcutaneous injection of buprenorphine (Buvidal).
* Liaise with community pharmacies to ensure prescriptions are in place and monitored appropriately. Provide advice, support and training to GPs, pharmacists and other professionals where appropriate. Participate in clinical audits and service reviews.
* Facilitate the transfer of clients from secondary to primary care services, and lead with referrals / liaison with Community Mental Health Teams (CMHTs) and other community substance use treatment providers on leaving Kaleidoscope services.
* Actively encourage people who access our services to get involved and give feedback, ensuring their views are considered in informing the development of the service.
* Collate and enter data electronically, and contribute to service reports.
Professional Development
Maintain and develop personal expertise and knowledge by reading, networking and further training in order to develop professional competence and to contribute to the development of competence within the team.
Help with the induction and orientation of new staff, volunteers and students.
Where relevant, develop and maintain the competencies needed to undertake the role of Substance Use Nurse.
Ensure professional development by seeking regular supervision and annual appraisals.
Effectively manage own time and prioritise workload, maintaining own administration and accurate personal records regarding travel, diary appointments, TOIL, and expenses incurred.
Be accountable and responsible for own professional practice, operating in line with professional code of conduct and within organisational policy and procedures.
Service Development
Ensure a high quality service is delivered in line with the service specification and regional delivery plans. Contribute to the delivery of internal teaching and training programmes.
Work collaboratively with colleagues to further develop the service.
This post has been identified as requiring the post holder to provide evidence of BBV immunisation status in order to carry out the role effectively. The role may require the post hold to undertake Dry Blood Spot Testing/ Substance Use Testing/ Working in Needle Exchange or is a clinical role and therefore Kaleidoscope wishes to ensure that staff are not at risk of infection or transmission of a disease that may affect their own or patient safety. Staff will therefore need to evidence their immunisation history, exemption or non-responder on commencement of employment or at routine intervals at the request of the Organisation.
Person Specification
Qualifications
* RGN/RMN with valid NMC Pin
* One year post- registration experience in Substance Misuse
* Independent/supplementary nurse prescribing qualification
* Training in Anaphylaxis and BBV
* Relevant training in Phlebotomy
* Relevant specialist qualification in alcohol screening and brief advice and substance misuse treatment
Other
* Full, current driving licence and use of a car which is insured for business use
* Prepared to undertake enhanced Disclosure and Barring Service (DBS) vetting
* Prepared to undertake vetting as an external contractor with South Wales Police
* Flexibility to work at multiple locations
* Develop and maintain the competencies needed to undertake the role of a Kaleidoscope nurse in accordance with the NMC Code of Professional Conduct and Mental Health Legislation, which incorporates and respects differences in gender, sexuality, ethnicity, religion, culture and disability, including being accountable and responsible for own professional practice, and within organisational policy and procedures.
Experience
* Excellent care co-ordination skills, with experience in undertaking comprehensive clinical assessment, care- planning and treatment reviews with people who have diverse and complex needs.
* Experience of using electronic case management / clinical notes recording system. E.g. PalBase / PARIS/ DELIUS
* Experience of treating individuals requiring alcohol detoxification
* Experience of developing and maintaining successful partnerships with GPs, treatment agencies and other healthcare providers.
* Experience in undertaking clinical audits
* Experience of delivering outreach and engagement model of treatment and care
* Knowledge of housing options pathways and experience of working with individuals at risk of homelessness
* Compliance: An insightful evidence- based approach to professional development and personal practice, including a commitment to clinical governance/quality assurance. Experience in undertaking clinical audits.
* Leadership: Demonstrable ability to help with the induction and orientation of new staff, volunteers and students
* Teamwork: Ability to build and develop relationships within the service, strategic partners and other external parties/ organisations; able to resolve conflict.

* Communication: Excellent written, verbal and presentation skills. Excellent IT skills: managing administration issues, monitoring and clinical activity data reporting. Good facilitation/negotiation skills and a commitment to working in partnership with other agencies.
* Commercial Awareness: A flexible approach to working in a developing service and a willingness to assist in its improvement and awareness of efficiencies.
* Customer Focus: A pro-active attitude regarding user involvement and clear ideas about how to involve service users in the planning, development and monitoring of service delivery.
* Planning and Organising: Demonstrable experience of managing tasks and deadlines; able to resolve conflicting priorities whilst demonstrating responsible time management and efficacy of prioritising workload.
* The post holder will have responsibility for promoting and safeguarding the welfare of vulnerable adults/ children (as appropriate) where the employee has responsibility or contact with the vulnerable person.
