Overview

As a Team Leader at Avery Healthcare, your focus will be to support the home management team in delivering high-quality, person-centred care. You will be responsible for implementing, monitoring, and evaluating care plans, assisting with smooth transitions of resident discharges, and leading in the monitoring of resident well-being. You will also be expected to practice safe systems of work and demonstrate the ability to lead staff effectively.

* Lead in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
* Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
* Lead with monitoring the well-being of each resident, be aware of any change impacting care and care planning, and ensure that changing needs are met and written records are amended to reflect any variation.
* Practice safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.


About You

Our ideal candidate must:

* Have an NVQ 3 or equivalent in care for care homes.
* Have two years experience working in a senior position with the resident group.
* Have the ability to communicate effectively both verbally and in writing.
* Demonstrate the ability to lead staff and act responsibly in the absence of the manager.

Please note this role will require:

* ADBS Disclosure check, the cost of which will be met by Avery Healthcare.
* Proof of eligibility to work in the UK.

Person Specification


Qualifications

* To be successful in this role, you must have an NVQ 3 or equivalent in care for care homes, and at least two years of experience working in a senior position with the resident group. You should also demonstrate the ability to communicate effectively both verbally and in writing, as well as the ability to lead staff and act responsibly in the absence of the manager.
