This role involves delivering compassionate care to support people to remain as independent as possible in their own homes. You will work as part of the Homefirst East Team in East Cornwall, a large rural area where public transport is not available. You must be able to travel independently to visits and have access to your own vehicle. Travel costs for journeys during work time are supported, and pool cars are available for work use, although availability cannot be guaranteed.
Responsibilities
* Provide a versatile and responsive service to optimise opportunities for patients’ reablement within the community setting.
* Assist patients to achieve their reablement goals using a range of specific activities, without direct supervision, supporting independence in daily living skills, personal care and normal functional activity with appropriate aids/equipment.
* Be part of an Integrated Care Pathway for older patients involving a range of services to prevent hospital admission and support discharge from hospital.
* Deliver compassionate care to patients in their own homes.
* Report any concerns or issues to the manager in charge and maintain accurate records using various IT systems.
* Make or receive telephone calls and communicate effectively with other professionals, patients and their relatives as required.
Qualifications and Working Conditions
* Excellent communication skills and a positive approach to people who may have communication difficulties or who have challenging and complex health needs.
* Previous experience in a community setting is necessary.
* Work a rota that covers seven days a week (including bank holidays) from 08:00 to 20:00 hours. Flexibility within the rota, and geographical location, will be important.
* Ability to work independently (lone working).
* Must be able to obtain a valid driving licence and have or be able to obtain a clean driving record to travel to and from patients’ homes.
