Job Description
As a Community Care Professional,you will provide essential care and support to clients in the community —enabling clients to maintain dignity, independence, and quality of life within their own homes;
Key Responsibilities
* Deliver high-quality, person-centred care in clients’ homes
* Support with daily living tasks such as washing, dressing, mobility, light household duties and meal preparation
* Provide companionship and build positive, trusting relationships
* Assist with medication prompts and general wellbeing monitoring
* Accompanying your client to appointments, or a visit to the shops
* Record and report care activities accurately
* Promote independence, respect, and choice in every interaction
* Communicate effectively with clients, families, and team members to ensure seamless care coordination
* Support clients in maintaining their independence and social engagement within the community
