Offer description

This post is Fixed Term for 9 months ending 26.03.2026 due to funding.

The postholder will work under the direction and supervision of the Lead Community Nurse or delegated Registered Learning Disabilities Nurse to assist with the provision of and access to Primary Care for People with Learning Disabilities, to contribute to a reduction in health inequalities.


Main duties of the job

Work directly with Service Users providing practical support in accessingPrimary Care services, including access to an annual health check by the GP as directed by the Welsh Government.

Provide information to Service Users on health promotion and well-being.Works autonomously with Service Users in community settings utilising a person-centred and empowering approach, respecting the individuality, values and cultural diversity of Service Users.

Liaises with other services/agencies as appropriate.

Organises own caseload/workload including all administrative tasks and data entry.

Records information related to the Service User in timely manner.

Reports any concerns or risks regarding Service Users to relevant staff.

Develops specialist knowledge of Primary Care resources within the area.

Demonstrates attitudes that ensure that the expressed needs and choices of the Service User are respected.

Person Specification


Qualifications

* Hold a recognized qualification in the field of Health and/or Social Care - GCF/NVQ Level 3 or equivalent
* Knowledge of current best practice within Learning Disabilities
* Knowledge of the health inequalities experienced by people with Learning Disabilitie
* Knowledge of national policy drivers regarding Learning Disabilities


Experience

* Experience of working in the field of Learning Disabilities
* Experience of direct Service User support
* Evidence of CPD
* Experience of working within a Community setting
* Experience of multidisciplinary/ multi-agency working
* Ability to work autonomously in community settings Ability to follow plans of care devised by registered professionals Ability to identify risk factors in the provision of support given to Service User Effective written and oral communication skills Computer literate
* Ability to speak Welsh
* Understanding of multidisciplinary/ multi-agency team dynamics and the need to maintain effective team working across professional and agency boundaries
