We have GP Vacancies at our Health Board Practice, West End Medical Centre, in Conwy County; West End Medical Centre is situated in Colwyn Bay, with a list size of 18,400 patients. Newly merged from to Practices within the same building, West End Medical Centre is focused on delivering a social model of care and has a multi-disciplinary team including GPs, Nurses, Physiotherapists and Pharmacists. The practice is also moving towards recruitment of other health care professionals such as Advanced Practice Paramedics and Physicians Associates.

Main Duties
In general, the post-holder will be expected to undertake the normal duties and responsibilities associated with GP work within primary care. In a coordinated manner within a managed practice timetable, the post-holder will make him/her available to take any duties including urgent consultations, telephone, video, and email consultations and queries, visiting patients at home, providing advice to other colleagues, and dealing with queries, paperwork, referrals and core responsibilities in a timely manner.
Make professional, autonomous decisions relating to problems, including undifferentiated and undiagnosed problems, self‑refer or referral from other healthcare workers within the organization, with due reference to best practice and evidence-based guidelines.
Screen patients for disease risk factors, early signs and promote public health interventions.
Admit patients, discharge, coordinate care and collect data for audit purposes.

Qualifications
* Full GMC Registration with a licence to practise and entry on the GP Register
* Certificate of Completion of Training (CCT) in General Practice / Certificate Confirming Eligibility for General Practice Registration (CEGPR), or equivalent
* Included on a Medical Performers List or eligible
* Evidence of continuous professional development
Desirable criteria
* MRCGP
* Academic excellence (prizes, merits, distinctions etc.)
Experience
* Experience in general practice and working in multi-disciplinary team
* Vocational GP training
Desirable experience
* Eligibility for minor surgery, obstetrics, family planning accreditation
Skills and Knowledge Ability
* Proven ability to handle a busy and varied primary care caseload and respond flexibly to workload fluctuations
* Ability to take independent clinical decisions when necessary and to seek advice from senior doctors as appropriate
* Commitment to team approach and multi-disciplinary working
* Effective counselling and communication skills
* Understanding of clinical risk management and clinical governance
* Commitment to participating in and understanding of the management process
* Good IT proficiency
