Sandbanks Ward - Oncology
Sandbanks ward is a 23-bedded unit specializing in oncology care. Our patients include those experiencing side effects from chemotherapy and radiotherapy, requiring brachytherapy, ablation, or symptom management. Due to the nature of their conditions, we also collaborate closely with hospital palliative care and end-of-life teams.
Discharge Coordinator Role
The primary responsibility of the discharge coordinator is to facilitate safe and smooth patient discharges from hospital. This involves planning, coordinating all aspects of the discharge process, and ensuring effective communication among the multidisciplinary team (MDT). The coordinator works closely with patients, families, healthcare professionals, and external agencies to ensure a positive discharge experience.

Key Responsibilities
1. Act as a clinically independent and autonomous practitioner, supporting the MDT in facilitating safe, effective discharges as part of the ward team and integrated discharge coordination team.
2. Provide expert advice to patients, families, carers, and staff regarding the Trust’s Discharge Policy, planning procedures, and processes for both simple and complex discharges.
3. Support Sandbanks ward as a discharge expert, liaising with patients, families, the ward MDT, and Single Point of Access (SPA) to ensure timely discharges from the trust.
