* You have a degree or relevant professional registration as a Nurse, Occupational Therapist, or Social Worker and are an active team member.

* Travel independently around the county is required, with a full UK driving license and vehicle. Public transport is not suitable for this role.

Key responsibilities include:
* Assessing individuals and families with complex health presentations and higher risk levels.
* Developing care plans collaboratively.
* Providing specialist assessments and advice.
* Managing ongoing risks collaboratively with service users and carers.
* Reviewing risk factors regularly and adjusting management accordingly.
* Engaging service users and families using assertive outreach approaches.
* Contributing to the development of the Trust nursing strategy.
* Ensuring evidence-based practice aligned with NICE guidelines and Trust policies.
* Supervising junior staff and trainees where appropriate.
* Acting within the professional code of conduct.
