Community Senior Practitioner
Role Responsibility
We are recruiting Community Senior Practitioners with diverse skills and experience for the Gravesend Community Team.
We operate a hybrid model of office-based and remote working to meet service needs.
£3000 Golden Hello payment for new joiners (subject to criteria) and ongoing service-related payments.

We support adults with various care needs, conducting assessments such as Care Act, Mental Capacity Act (MCA), Court of Protection applications, and s42 safeguarding enquiries.
Our interventions are based on excellent practice, innovation, and meaningful measures, emphasizing relationship-building with individuals, families, and communities.
About You
We welcome applications from experienced social workers with post-qualification experience in Social Services, Health agencies, or related organizations.
You must hold registration with a professional body (Social Work England, HCPC, Nursing and Midwifery Council) and be competent at the Advanced Practitioner level.
Experience in managing risk and safeguarding enquiries is required.
Demonstrated expertise in staff supervision, development, and performance management is essential.

Flexibility to travel across the area at various times is necessary.
