Job overview

We are looking for a dynamic individual who is confident in their clinical practice and who can drive forward service transformation inline with the national agenda.

The post holder will work within the York and Selby Community in-patient units as part of Community and Care of the Elderly Services.

Main duties of the job

The aim and main function of the community in-patient unit bed base is to provide short term rehabilitation, reablement, recovery and end of life care to the patients.

The post holder will practice as an Advanced Clinical Practitioner within the Community Inpatient Units, they will join a Team of Advanced Clinical Practitioners and Trust Grade Doctor to provide patient-centred, high-quality care.

The role will encompass the skills of assessment, examination, diagnosis and treatment within this area pf practice. Supporting admission from the Hospital and the community and timely patient discharges from the in-patient units.

Qualifications

Essential criteria

8. Registered practitioner (NMC, HCPC)
9. Evidence of a completed MSc ACP or portfolio of practice accredited by the Centre for Advancing Practice
10. Non-medical prescribing
11. Resuscitation Council (UK) Immediate Life Support Provider
12. Teaching and assessing qualification/ mentor qualification
13. IRMER certificate

Desirable criteria

14. PG Diploma in Geriatric Medicine
15. Completion of an Internal Medicine course

Knowledge & Experience

Essential criteria

16. Management, support and leadership of trainee ACPs
17. Experience of mentorship, supervision and teaching
18. Highly developed, specialist clinical skills required for area of work
19. Expert knowledge in clinical field
