Job overview

The post holder will be a practicing midwife who will lead and manage the PMRT (Perinatal Mortality Review Tool) review processes for all eligible cases of intrauterine and neonatal deaths.

The post holder will be responsible for identifying applicable cases, coordinating and managing the reviews, share learnings and implement changes to improve practice, in line with standards set by MBRRACE, NHS Resolution Clinical Negligence Scheme for Trusts, and the National Bereavement Care Pathway.

Main duties of the job

The PMRT Midwife will be an expert who is passionate about improving maternity services and promoting patient safety. They will be responsible for ensuring that all eligible cases of late miscarriage, stillbirth and neonatal death are reviewed in line with national standards and local governance strategy using the MBRRACE tool and be responsible for report writing to a high standard in collaboration with the PMRT consultant lead.

o They will be responsible for working closely with the bereavement midwives to ensure all cases are identified for review and notified to MBRRACE. The bereavement midwives will be responsible for completing the MBRRACE notification and perinatal surveillance. The PMRT Midwife will then take over the responsibility of the case.

o The post holder will be responsible for ensuring that the PMRT review process is implemented to meet standards set by the NHS Resolution Clinical Negligence Scheme for Trusts (CNST).

o They will work closely with the Perinatal Quality and Safety Team and the bereavement midwives to support and facilitate the PMRT process and to make sure that the families’ voice is heard and included at each Perinatal Mortality Review.

o They will maintain responsibility for organising the PMRT multidisciplinary review meetings, including inviting external PMRT teams to maintain a fair, non-bias review.

Detailed job description and main responsibilities

o The PMRT Midwife will complete the tool to an expected standard and will collate thorough timelines of events to be used to review individual cases and will present these at the PMRT MDT review meetings.

o They will build close contacts with external agencies such as the Maternity and Newborn Safety Investigations (MNSI) Programme, other NHS trusts, Coroner and hospital mortuary staff to ensure all information required for PMRT review is obtained. They will also be the point of contact for internal and external teams for queries related to PMRT, including local teams or external teams organising joint PMRT reviews.

o They will work with the PMRT consultant lead to inform families of review findings, in a timely manner. Families will be offered the opportunity to meet face to face with the PMRT Midwife and Consultant Lead so they have an opportunity to ask any questions.

o They will work with relevant colleagues to make sure that information from PMRT is shared with the NCL Child Death Overview Panel (CDOP).

o The post holder will work closely with the Perinatal Quality and Safety Team to create appropriate action plans for the care issues and learnings identified at PMRT reviews. They will work with the team to keep up to date with the action log and support ongoing learning, communication, and improvement across the service.

o They will lead on developing a planned programme of work to achieve the recommendations, including quality improvement projects to support embedding learning from PMRT. They will also collaborate with other specialist midwives on updating and developing policies and guidelines and delivering training.

o They will be responsible for producing quarterly PMRT reports, in line with CNST requirements, and will share and present these to the required division and boards.

o The PMRT Midwife will work with the bereavement midwives to audit and analyse the data captured by MBRRACE and PMRT and disseminate the annual perinatal mortality reports, including presenting at Women’s Services Audit Meeting.

o The may also support in responding to complaints within the division by sharing findings from the PMRT review with the complaints lead.

o The post holder must be able to work collaboratively but also be able to manage their own time effectively and be able to prioritise to meet deadlines.

Professional

o Work in accordance with the NMC’s rules and standards for midwives. Ensure that other NMC documents/standards and Trust policies and guidelines are adhered to. Maintain and develop professional knowledge and competence in accordance with the NMC recommendations

o Ensure high standards of patient care are monitored and maintained by participating in quality assurance programmes.

o Demonstrate good interpersonal skills and understand the need to communicate effectively with the women, their families, colleagues and multidisciplinary teams.

o Work towards the aims and objectives of the Trust’s Nursing and Midwifery Strategy.

o Communicate sensitive information to appropriate team professionals/agencies/patients using discretion and always maintaining confidentiality.

o Provide emotional support to bereaved families or those who have undergone difficult and traumatic experiences during childbirth.

o Plan, organise and participate in Parent Education in the ward, department and clinics.

o Follow Trust and Maternity Unit Guidelines and participate in the Practice Review and Guidelines groups

o Keep up to date with changes and developments in midwifery care and co-operate in any research projects applicable to the maternity unit.

o Knowledge of the risk management strategy for the maternity service and to ensure that risk trigger events are reported accordingly by using Radar

o Child Protection: Knowledge of all issues and when appropriate initiate the referral to social services. Liaise closely and actively participate with social services and other team members.

o Liaise with medical staff as necessary and report any abnormal condition. Liaise with the Supervisor of Midwives for professional issues, which affect the safety of mother and baby.

Clinical

o To provide women-centred care, individualised to the needs of the mother and her family. Acting as a pregnancy loss expert in cases affected by pregnancy loss supporting colleagues in their delivery of care

o Understanding the national bereavement pathway framework and what is required clinically for bereaved parents and families

o Provide care in labour and assessing both mother and baby post birth. Ensuring necessary paperwork is complete at the time.

o Identify and report any changes from the ‘norm’ or emergency situations to senior midwife or obstetric team

o To provide midwifery expertise and advice, working as an autonomous practitioner accountable to the senior midwife

o Able to support completion of essential paperwork for registration of a stillbirth or early neonatal death

Leadership

o To participate in training or observing in the maternity unit

o To be responsible for the orientation of staff attending the unit from all areas of the multidisciplinary team

o To provide telephone or direct advice on maternity and midwifery care to clients and professionals when necessary, and act as a resource for staff when working with women who have had a pregnancy loss.
