We are working closely alongside a Local Authority in Cardiff to assist with the appointment of a Reablement Care Worker to work in a residential care setting.
We have both regular shifts (18.50 hours per week) and bank shifts available. Please note that we don't offer sponsorship, and the worker should be able to work full time without restrictions. We would need 2 years work references checked out before you start the shifts.
Rate of Pay: £14.09 - £18.42 per hour

Summary
The Reablement Care Worker joins the Community Resource Team (CRT) Homecare, a service operating throughout Cardiff that focuses on preventing unnecessary hospital admissions and facilitating discharges. The primary goal is to assist citizens in living independently within their own homes. Working closely with health colleagues, the post holder supports service users in achieving their specific reablement and wellbeing goals through tailored service delivery plans.
Responsibilities
* Assist clients in achieving their individual reablement and wellbeing goals as defined in their assessment
* Maintain high standards of observation and recording, which are essential for tracking service user progress
* Commit to the safety and protection of vulnerable adults in line with the Councils safeguarding and child protection priorities.
* Partner with health professionals and other colleagues to ensure cohesive support for the service user.
* Work according to a rota, which may include various shifts such as evening and weekend work to meet service needs
Qualifications
* Must have the ability to train for and achieve the relevant Health and Social Care (HSc) level required for the post (previously NVQ/QCF).
* The post holder is required to register with Social Care Wales

* Ability to use a mobile device to manage and update daily task allocations.
