The post holder will be required to provide effective and timely care within a multidisciplinary team setting for the Older People Community Mental Health Team (OPCMHT) who present with both organic and functional mental health problems, their carers and families and others who are significant to the individual.


Main duties of the job

The post holder to demonstrate expert practice in the care of clients with mental health needs and maintain clinical expertise. taking responsibility for initiating risk assessment, mental capacity, best interest assessments, crisis planning, dialog plus and core assessment, ensuring the care and interventions provided within the ISS are evidence based and in accordance with NICE guidance.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.


Job responsibilities

To provide mental health training to the Trust and external agencies, taking responsibility for planning delivering, implementation and evaluation of training.

To have a good working knowledge of community resources for older adults with mental health needs and their carers to access, to signpost, provide advice, education and support.

Be responsible and accountable for the management of own caseload as directed by the OPCMHT manager.

To be highly skilled in working with potential aggressive behaviour of clients in the community.

To develop care plans in collaboration with the patient, carer, and other relevant person/agency.

To provide skilled assessments of patients and carers emotional and psychological needs based upon the understanding, interpretation and integration of data given by the patient and other sources.

To conduct ongoing assessments of the impact of prescribed psychotropic medication on individual patients both in terms of therapeutic and untoward side effects.

Maintain contact with service users on their caseload during periods of admission, including attendance at MDT meetings. To take lead in the planning and implementation of the clients re-integration back into the community.

Perform thorough physical health assessments, encompassing monitoring of vital signs, weight, blood pressure, blood sugar and other relevant indicators to ensure comprehensive health evaluation.

Occupational Therapists To conduct comprehensive assessments of clients referred to the OPCMHT, focussing on occupational therapy equipment needs, telecare needs, including aids for the home use and mobility assessments including wheelchair assessments.


Person Specification

* oBSc (Hons) in Occupational Therapy
* oEvidence of further post-registration education and training
* oPost registration experience as a Band 5 OT preferably in a mental health setting working with older adults with functional and organic mental health needs.
* oAbility to care co-ordinate within dialog plus requirements
* Ability to demonstrate transferable skills if currently working in another setting.
* Experience of working in a community mental health setting. Experience/Interest in working with older adults
* Safeguarding Adult Processes
* oSpecialist knowledge of statutory policy relating to mental health care delivery incl. Dialog plus, National Service Framework for Mental Health, Mental Health Act and Mental Capacity Act
* Further training within the specialist area of Mental Health for the Older Person.
* Ability to demonstrate transferable skills if currently working in another setting.
* Experience of working in a community mental health setting. Experience/Interest in working with older adults
* Safeguarding Adult Processes


Interview

* oAbility to undertake comprehensive risk assessment and implement risk management plans.

£45,953 to £54,254 a yearper annum inclusive of HCAS
