The Dudley Group NHS Foundation Trust invites applications for a Band 6 Specialist Practice District Nurse to join its Brierley Hill District Nursing Team. The post-holder is responsible for the ongoing assessment of care needs, and the development, implementation and evaluation of programmes of care. The post-holder will carry out all relevant forms of care and take responsibility for caseload holders in their absence. Management of the work area, clinical caseload, supervision and development of staff is also key.
Responsibilities
* Assess and evaluate patient care needs.
* Develop, implement and evaluate care programmes.

Qualifications
* First-level Registered Nurse.
* Nurse Prescriber (or registration as V150/V300).
* Degree in District Nursing or equivalent, with a Specialist practitioner qualification in District Nursing.
* Proof of continual professional development.
* Specialist Community Practitioner – District Nursing qualification.
Knowledge & Training
* Demonstrated knowledge of research‑based practice.
* Knowledge of major NHS issues.
* Post‑registration experience including mentoring, project work, specialist training, management and development of others.
* Willingness to undertake Band 6 competencies.
* Understanding of primary care issues.

* Significant post‑registration experience.
* Experience working in primary care and leading a community nursing team.
* Experience supervising qualified and unqualified staff.
* Experience identifying development needs of others and giving effective feedback.
* Experience in inter‑agency and collaborative working.
