Social Workers in the neighbourhood team work closely with health and community colleagues to understand community resources that facilitate independence, helping individuals remain in their own communities using a strengths-based approach.
Responsibilities include:
1. Working in the office at least 2 days per week, with the rest of the time supporting a hybrid working model using technology for communication and support.
2. Utilizing extensive skills and experience to support and empower residents of Somerset to stay independent longer.
3. Tailoring solutions to residents' needs, providing continuous support and intervention.
4. Promoting independence in all team activities.
5. Collaborating with Health and Social Care colleagues as part of the neighbourhood PCNs.
6. Liaising with key partners to improve service provision, team training, and recruitment.
7. Contributing to the development and shaping of integrated local care services.

This is a Grade 10 position with a starting salary of £37,280, rising to £41,771 per annum. Appointments are typically made at the lower end of this scale.
