You will provide a high quality caring support service to the most vulnerable patient within the Trust supporting the Trust to meet their complex needs.
With this in mind an exciting opportunity has arisen within the enhanced support team to provide support to a range of specialist practitioners who work with some of our most vulnerable patients across adults and childrens services. We are looking for a diverse individual who is looking to gain a range of experience that will help you in a future career.
This role is only available to those who are 18 and over due to the type of care the team deliver.
Main duties of the job
The post holder will work within the innovative Enhanced Support Team providing care for a range of patients in different clinical areas within the hospital who may need a higher level of physical and psychological support.
This will include people living with dementia, suffering from delirium, patients who are at risk of falls, or who may need additional support or care due to other physical or mental health needs.
Successful candidates will receive additional training with an emphasis on dementia, delirium, mental health, communication skills and reminiscence therapy.

Person Specification
Qualifications and Training
* Education to GCSE standard (or equivalent), to include Maths & English
* QCF level 2 Clinical Healthcare Support or willing to work towards and complete within 12 months
* Care Certificate (or willing to work towards)
* Health Related Training, First Aid, Moving & Handling, Food Handling.
Experience
* Understands the need for confidentiality
* Understands & committed to a caring approach to patients, relatives/carers
* Experience in a NHS/health care setting.
* Experience working in a surgical/Medicine/emergency care setting.
Skills and Knowledge
* Able to demonstrate a caring approach to patients & relatives & works so that patients' dignity & respect is maintained at all times.
* Able to communicate routine information including observations effectively verbally & written to staff, patients/relatives.
* Able to prioritise own workload.
* Able to work under own initiative within boundaries of role.
* Demonstrates awareness of importance of working as part of a team.
* Basic understanding of nursing practice & code of conduct
