[311-R402-26] Living Well Mental Health Practitioner
The postholder will work as a clinical practitioner as part of the multi-disciplinary team in the Living Well Hub. The Living Well Hub forms part of the transformed community mental health service in the borough as per the national Community Mental Health Framework for Adults and Older Adults guidance and the GM Living Well Handbook.
The transformed service will manage the needs of people with SMI over the age of 18 (adults and older adults) in the borough without the need for repeated referrals and assessments and ensure no patients are discharged to no support. The Hub will work in partnership with PCNs, social care, VCSFE and specialist mental health teams to ensure patients receive the right care at the right time from the right agency.
The role of the postholder will be to give advice, triage, and where indicated to clinically assess and provide short term support for people experiencing complex mental health difficulties and if indicated to transfer patients to the appropriate service in the transformed community mental health service.
The postholder will work to and comply with all Pennine Care Trust Policies and Procedures.

Main Duties and Responsibilities
Clinical Work
•
To work with Hub partners to triage all referrals for adults and older adults, made to the service and facilitate onward transfer to the most appropriate service to meet the individual needs of the patient either within the Hub, to VCSFE partners, specialist MH teams or other partners such as housing, debt management.
•
Where indicated to carry out a clinical bio-psychosocial assessment of the service users’ current mental health needs and individual needs, including a risk assessment.
•
To identify where patients needs are too complex to manage in Primary care and to ensure transfer to specialist teams.
To work collaboratively with patients and carers to produce care plans which are personalised and goals based building on strengths not deficits, and promoting self care and recovery.

To provide short term psychotherapeutic interventions as appropriate linking in with keyworkers regarding patients care plan and outcomes.

To provide onward transfer to CRHT where patients need a same day response
•
To facilitate fast track step up in care for patients whose mental health is deteriorating.
•
To facilitate step downs from specialist community mental health care back into the hub

Education / Qualifications
•
Registered practitioner RMN/RNMH/Social Worker/OT with current registration
•
Evidence of on-going CPD in a relevant field

Short courses and/or experience to post graduate diploma level

Degree - or working towards completion.

Non-medical prescribing qualification

Further training in brief psychosocial interventions

Completion of Preparation for Mentorship / Practice Teacher / Fieldwork Educator qualification or willingness to complete
