An opportunity for the post of Senior Practitioner has arisen for the SCOPE team which is a specialist team working with individuals aged 14+ going through school transitions; whilst also supporting individuals across their whole life span.
The individuals we support have a diagnosed learning disability and/or autism and are identified as having complex care needs. * O – Offering young people and adults’ choice in their care packages
* P – Person-centred planning
* E – Enriching people’s lives.
The principal role of the SCOPE team is to provide life-long support for complex cases of individuals who reside across Perth and Kinross.
The team is made up from disciplines from both Health and Social Care, such as social workers, Outreach Workers, Psychology and aligned to NHS services that include Speech and Language Therapist, Physiotherapy, Dietetics, Psychiatry and Community Learning Disability Nurses. The team will link with local partners such as Education and Children’s services, Housing, other Health Services and third sector partners.
