Role details
The Hospital at Home (H@H) service based at Lorn and the Isles Hospital covers the Oban and Lorn area. It is a developing service that has proven to be a safe alternative to hospital admission. The service provides acute care at home with the objective of reducing hospital admission and reducing length of stay in hospital through expanding clinical pathways. It involves management of patients with both acute and long term conditions across a number of different clinical pathways, where their medical conditions can be managed safely and effectively at home. The team has received funding to expand the service and we require a Health Care Support Worker (HCSW) to support the clinical staff with clinical tasks to ensure smooth running of the service and provide appropriate care to patients. The post will be fixed term or possibly a secondment opportunity until March 2026.
