Main duties of the job
* You will undertake a variety of duties including face to face, telephone, and video consultations, home visits, prescriptions and queries and correspondence to the practice.
* Work with the practice quality and clinical leads to ensure we are providing a high standard of care
* Manage patients with undifferentiated and undiagnosed illnesses
* Develop care plans in line with local and national guidance following practice policies
* Adhere to practice protocols for chronic disease management
* Complete quality audits
* Prescribe in accordance to practice and local guidelines
* Be a part of the annual appraisal process to further develop your skills and knowledge

Job responsibilities
The following are the core responsibilities of the Salaried GP indelivering health services.
a. The delivery of highly effective medical care to the entitledpopulation
b. The provision of services commensurate with the primary care contract
c. To clinically examine and assess patient needs from a physiologicaland psychological perspective and plan clinical care accordingly referringpatients to other services/agencies in a timely manner, utilising Practiceand any other local guidelines
d. Generic prescribing adhering to local and national guidance
e. Prioritise health problems and intervene appropriately to assist thepatient in complex, urgent or emergency situations, including initiation ofeffective emergency care
f. Effective management of long-term conditions
g. To support patients to adopt health promotion strategies that promotehealthy lifestyles and apply principles of self-care
h. Processing of administration in a timely manner, including referrals,repeat prescription requests and other associated administrative tasks
i. Review medications following the appropriate policies, NICE/localclinical guidelines and local care pathways
j. Maintain accurate clinical records in conjunction with good practice,policy and guidance
k. Work collaboratively, accepting an equal share of the practiceworkload including being part of the on-call and home visit rota.
l. Adhere to best practice recommended through clinical guidelines andthe audit process
m. Support the delivery of anticipatory care plans
n. Contribute to the successful implementation of continuous improvementand quality initiatives within the practice
o. Accept delegated responsibility for a specific area (or areas) or theQOF
p. Attend and contribute effectively to practice meetings as required
q. Contribute effectively to the development and maintenance of thepractice including clinical governance and training

1. Participate in the spectrum of clinical governance processes
2. Participate in formal training events promoting best practice in area of expertise Provide leadership and education for members of the multi-disciplinary team, providing guidance and support when necessary
3. Participate in local initiatives to enhance service delivery and patient care
Person Specification
Experience
* Ability to both work autonomously and as part of a multi-disciplinary team
* Experience of working in a primary care environment
* Experience of providing home visits
* Minimum of two years as a salaried GP
* General understanding of the primary care contract
* Proven ability to evaluate the safety and effectiveness of own clinical practice
* An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners
Qualifications
* Qualified GP
* MRCGP
* Vocational Training Certificate or equivalent JCPTGP
* Certificate of Completion of Training (CCT)

Clinical knowledge and Skills
* Outstanding level of clinical knowledge and skills commensurate with that of an experienced GP
* Understanding of the importance of evidence-based practice
* Good MS Office and clinical system (EMIS), IT knowledge
* Understanding of the spectrum of clinical governance processes
* Excellent communication skills (written and oral) including recording of accurate clinical notes
* Ability to work within own scope of practice and understand when to refer to GP colleagues
* Experience of triage and the ability to effectively assess and diagnose
* Ability to listen whilst being honest and caring and be both sensitive and empathetic in distressing situations
* Understanding of the local pathology tests processes and the ability to process the results and advising patients accordingly
* Ability to assess and manage acute deteriorations /exacerbations of long-term conditions
* Understanding of safeguarding adults and children
* Understanding of the importance of evidence-based practice
* Knowledge of health promotion strategies
* Experience of medicines management
* Knowledge of public health issues in the local area
* Understanding and knowledge of QOF and enhanced service
