In this role, you will be responsible for actively identifying patients in the Emergency Department who are likely in their last year of life, providing specialist palliative care and facilitating their prompt discharge to appropriate care providers, aligned with patients’ preferred place of care. Primary job responsibilities include: Lead on clinical assessments and care planning Provide advanced symptom control and communication support Coordinate with community and hospital palliative care services to facilitate rapid discharge home/preferred place of care Supervise Senior Healthcare Assistants, mentor emergency departments staff, and support team learning You will also manage nursing services for individuals with terminal illnesses, supporting their loved ones, and collaborating with health professionals. As a vital member of our multidisciplinary team, you will offer expert advice and support, managing a caseload of patients with complex needs while ensuring the highest standards of care. Job Structure: Fixed term contract until early 2027, with possible extension Hours: 37.5 per week, long day shifts Workdays: Starts Monday to Friday; includes weekends as the service grows Salary: Marie Curie Clinical Pay Scale Band 6 (AFC aligned) £38,682–£46,580, Plus High-cost area supplement Location: North Middlesex University Hospital, London What we’re looking for: Experience working in Specialist Palliative Care Proven experience in providing care to patients with complex needs, ensuring comfort and quality of life during terminal illness. Prior experience managing complex discharges within an emergency department is preferred but not required. Registered Nurse on NMC register (Adult) Must be a registered nurse with the Nursing and Midwifery Council, ensuring adherence to professional standards and regulations. Ability to connect with patients and their families on an emotional level, providing support and understanding during difficult times.
