Social Worker – Older People & Physical Disabilities (Ormskirk, West Lancashire)
Salary: £36,363 – £40,778 per annum

Team: Older People and Physical Disability Team, West Lancashire (Based in Ormskirk)

Responsibilities
As part of our Adult Services Older Adults and Physical Disability Community Teams, you will undertake strength‑based, outcome‑focused assessments and support planning, ensuring that residents receive the right level of support. You will also support people to access local services, maintain independence, promote health and wellbeing, and live independently. A key part of the role includes working alongside other agencies, professionals, and informal carers to provide a holistic service.
Qualifications
* Registered with Social Work England (SWE) or have applied to register with SWE.
* Will be required to apply to the Disclosure and Barring Service (DBS) for a ‘disclosure’; further information can be found in the ‘Our Recruitment Process’ section.
* Must provide a car for use in connection with the duties of this post and be insured for business use (exception if unable to drive due to a disability).
