We have an exciting opportunity to join a growing, supportive MedicinesManagement Team in our small PCN with opportunities for personal development,impactful quality improvement and patient facing clinics!

We have been impressed by the impact that the Pharmacists and PharmacyTechnicians have had across Lancing and Sompting PCN and would like to expand ourteams capacity.

The population is growing, though currently stands at 29,000.There are 3 surgeries and currently 3 PCN Pharmacists and 4 Pharmacy Techniciansthat work closely with the wider Practice and PCN Teams.

Our PCN strives to provide our patients with excellent clinicalcare and motivational support delivered by warm and caring clinical staff.

We have excellent relationships with the local Community Pharmaciesand Community Teams.

Specialist Interests, Experience and Independent Prescribing Qualificationsare welcomed, though not essential, especially around Chronic Disease Managementincluding Frailty.


Main duties of the job

A key role in GeneralPractice combining patient-facing, analytical and administrative elements of work, bringing expert knowledge ofmedicines to multi-disciplinary primary care teams.

Job responsibilities

The role of the Clinical Pharmacist will be made up of many of the following:

Clinically assess and treat patientsusing expert knowledge of medicines for specific diseaseareas.

Be prescribers, or committed to completing trainingto become prescribers.

Contribute to the deliveryof effective daily processes for the transferof care between hospitalsand GPs including effective signposting, recommendation of medication changesand communication accordingly.

Support colleagues and patients with medication queriesand monitoring.

Take responsibility for the care management of patients with chronic diseases.

Undertake clinical medication reviews to proactively manage people with complex polypharmacy.

Focusespecially on the elderly, peoplein care homes, those with multiple co-morbidities (in particular Frailty, COPD and Asthma) and people with LearningDisabilities or Autism.

Provide specialist expertise in the use of medicines, while helping to address both the publichealth and social care needs of patients.

Through the use of audit and action planning,help to tackle inequalities.

Provide leadership on person-centred medicines optimisation (including ensuring prescribers in thepractice conserve antibiotics in line with local antimicrobial stewardshipguidance) and quality improvement.

Contribute to the General PracticeQuality and OutcomesFramework (QOF) and Enhanced Services.

UseStructured Medication Reviewsto support patientsto take their medications to get the bestfrom them, reduce waste and promote self-care.

Contribute pharmaceutical advice forthe development and implementation of new services that have medicinal components (e.g. adviceon treatment pathwaysand patient information leaflets).

Workwith general practiceteams to ensurethat practices are compliant with CQC standardswhere medicines are involved.


Person Specification


Qualifications

* Master of Pharmacy or equivalent
* Registration with General Pharmaceutical Council
* Be willing to commit to or have completed the CPPE Pathway
* Be willing to complete Independent Prescribing Qualification
* Experience of Common Acute and Long-term Conditions
* Work effectively independently
* Excellent interpersonal, influencing and negotiating skills
* Rigorous in following legal, ethical, professional and organisational policies and codes of conduct
* Committed to continuous professional development
* Ability to apply research evidence base into the workplace
* Excellent organisational skills
* IT literate
* More than 2 years Experience as a Pharmacist
* Experience of Medication Reviews relevant in General Practice
* Track record of improving work processes
* Previous experience of working in a GP Surgery
* Up to date knowledge of CQC Regulations in General Practice
* Good understanding and history of quality improvement work through audit
* User of EMIS Web
