Job Title: Complex Care Case Manager (Non-Clinical)
Location: Denton
Salary: Up to £35K depending on experience plus bonus and on-call payment
Contract: Full-Time, Permanent
We’re looking for a Complex Case Manager who can make a real difference for our clients and foster great morale within their care team. Responsibilities
* Take ownership of a caseload of complex clients with clinical interventions (e.g., spinal injuries, acquired brain injuries, motor neuron disease, multiple sclerosis) and manage their care packages holistically.
* Coordinate and lead the transition, start-up, and continuity of all complex care packages; involve the client and MDT; manage the clinical training needs of the care team and maintain appropriate skill mix for safe and effective clinical practice.
* Uphold high standards of care with a person-centred approach.
* Prioritise individual wellbeing in care practices.
* Act as a responsive advocate for complaints and safeguarding, encouraging continuous improvement.
* Develop comprehensive care plans and risk assessments.
* Provide support for care packages through observations, mentoring, and regular reviews.
Qualifications & Requirements
* Passionate about putting people first.
* Strong organisational and time-management skills.
* Good understanding of CQC requirements.
* Experience managing complex care packages with clinical interventions (e.g., spinal injuries, acquired brain injuries, motor neuron disease, multiple sclerosis).
* Experience managing a team of healthcare workers.
* Driver with access to own car for work.
