Job summary
As an advanced practitioner this post holder will work collaboratively across the PCN to play a crucial role in clinical governance, building capacity and capability and providing resolution and critical thinking to some challenging patient diagnosis or issues.
Whilst operating at an advanced practitioner level you will drive forward service improvements and clinical governance alongside seeing patients face to face and doing care homes ward rounds and home visits. The PCN will benefit from your level of knowledge, skills, experience, and leadership to build capacity and capability across the network.
Main duties of the job
* To assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
* To manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action.
* To use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
* To actively take a personalised care approach and population centered care approach to enable shared decision making with the presenting person.
* To complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
* To provide specialised clinical knowledge to assess patients, order and interpret diagnostics and initiate treatment.
* To provide skilled team leadership to clinicians within the wider Multidisciplinary team.
* To provide a high level of clinical expertise.
* The post holder is an autonomous practitioner who will manage a caseload of patients, using evidence-based practice/patient focused principles to assess, plan, implement and evaluate interventions.

GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN). Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre and the Ryan Medical Centre. We serve a community of around 30,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.
As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024. This includes the following areas of focus: anticipatory care, cardiovascular disease prevention and diagnosis, enhancing health in care homes, early diagnosis of cancer, tackling neighbourhood inequalities, structured medication reviews, social prescribing reviews.

Key duties and responsibilities

* To support and undertake audit, research projects, and recommend changes in practice within specialist clinical area.
* The post holder is an autonomous practitioner who will manage a caseload of patients, using evidence-based practice/patient focused principles to assess, plan, implement and evaluate interventions.
* To ensure Shared Decision Making (SDM) is embedded within the service and evidenced.
* To communicate appropriate complex information effectively to patients and/or carers where there may be barriers to understanding.
* To inform them of the benefits and risks of suggested treatment options and to undertake this with groups or individuals.
* To demonstrate a sound reasoning of clinical governance, clinical risk and to demonstrate the use of treatment plans with an evidence-based link.
* To use advanced clinical reasoning skills enabling formal supervision, specialist support and advice for the AARS roles on a regular basis and be available to advise other staff on the management of complex patients as appropriate, in area of specialist expertise.
Building capacity and leadership
* To provide clinical leadership to the AARS team, provide 1:1 and carry out appraisal and training plans when needed.
Management of workload and personal development
* Act on own initiative in prioritising and managing workload, delegating tasks appropriately and referring to MDT Members in respect of clinical, managerial, and administrative areas, as necessary.
* Enrolled into review and appraisal systems within the PCN.
* Maintain your own personal and professional development as agreed at your annual review.
* Manage own time, workload, and resources effectively.
Collaborative working relationships
* Recognises the roles of other colleagues within the organisation and their role to patient care.
* Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders.
* Demonstrates the ability to work as a member of a team.

* Develop and maintain effective working relationships within and outside the PCN and locality.
* Foster and maintain strong links with all services across the locality.
* Explore potential for collaborative working and initiate/sustain relationships.
Education, Training and Development
* Attend training and events organized by Practices/PCN or other agencies.
* Participate in clinical and Practice/PCN meetings.
* Demonstrate mentoring and role modeling within the team.
* Participate in delivery of formal education programmes when needed.
* Ensure appropriate clinical supervision is in place to support development.
Additional information
Contribution to the implementation of services:
* Apply practices/PCN policies, standards, and guidance.
* Discuss with other team members how policies, standards and guidelines will affect own work.
* Undertaken any additional duties appropriate to the post as requested by the Partners/Practice Manager/PCN Manager and Clinical Director.

In the course of seeking treatment, patients entrust us with sensitive information. The post-holder should always respect patient confidentiality and not divulge patient information unless sanctioned by policy.
The post-holder will comply with data protection requirements and ensure appropriate handling of personal data.

The post-holder will recognise the importance of effective communication within the team and with patients and carers, including providing information in accessible ways.
Safeguarding Children and Vulnerable Adults
The PCN is committed to safeguarding. The post requires knowledge of safeguarding procedures and access to supervision and guidance from safeguarding leads. All employees must familiarise themselves with local safeguarding policies.
Special Working Conditions
The post-holder is required to travel between practice sites and other locations, attend meetings, and may work flexible hours including evenings, bank holidays and weekends if required.
Qualifications
* Masters degree in relevant area or equivalent Centre for Advancing Practice accreditation or ePortfolio route, with eligibility for advanced digital badge where applicable.
* Registered clinical practitioner with the relevant professional body
* Independent prescriber
* Knowledge of Caldicott Principles, Data Protection Act 2018 and safeguarding requirements
* Experience in primary care or General Practice
* Evidence of service improvement involvement and CPD commitment
* Good IT skills and ability to learn new systems quickly
* Willingness to travel to PCN sites and undertake home visits
