Overview
Role Summary: As a Nurse Associate you will be responsible for working clinically to deliver care to patients in their own homes and in a clinic environment. You will provide holistic care, advice and information to patients and carers within the home environment or other appropriate care settings.
Responsibilities
* Provide clinical care to patients in home and clinic settings.
* Deliver holistic care and information to patients and carers.
* Work with the team to prevent hospital admissions where possible and facilitate discharge for patients to be cared for at home.
* Strive to support high quality patient care that maximises independence and quality of life in the most appropriate environment.
Requirements / Qualifications
* Experience in community care is desirable but not essential; full training will be provided and you will be supported to develop in the role.
* Full driving licence and access to a car with business insurance to undertake the role.
* Lone working is expected after a period of induction with ongoing support from experienced nursing staff.
Working Arrangements
