Main duties of the job Join our Community Nursing Team delivering high-quality care to housebound patients in their own homes. As a Band 5 Community Staff Nurse, you will manage a defined caseload, using your clinical knowledge and skills to provide safe, effective, and patient-centred care. You will undertake holistic assessments of routine and complex patients, planning and delivering care in partnership with patients and their families. Working largely independently, you will carry out daily visits, document care accurately, and escalate concerns appropriately.
