With prior experience from the Local Authority or private care, you can join the multidisciplinary team at Aberbeeg Hospital in Abertillery, Wales, where you will provide care for men who require low and medium secure mental health services.
As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitate group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future. You will be the Safeguarding Lead, handling Safeguarding investigations and the discharging process, while enjoying access to a range of training opportunities that can help your career grow.

Key Responsibilities
* Lead safeguarding concerns involving adults and children, ensuring concerns are escalated and managed appropriately.
* Undertake complex assessments covering risk, social need, public protection, relapse prevention, and family/social context.
* Contribute to and co‑produce care plans with service users, carers, and the multi‑disciplinary team (MDT), ensuring the social perspective is embedded.
* Manage and prioritise a caseload of individuals with complex mental health, financial, behavioural, and forensic risks.
* Support, inform, and involve families and caregivers.
* Promote social inclusion and financial stability, whilst assisting with benefit claims and liaising with the DWP.
* Work as part of a well‑resourced MDT including Psychiatry, Psychology, Occupational Therapist, Nursing, and other specialists.
* Attend ward rounds, managers meeting, MAPPA panels, completing reports and discharge planning.
Requirements
* A Social Work degree.
* Experience within a similar environment.
* Experience working with vulnerable adults.
* Registered with Social Care Wales.

Aberbeeg Hospital, Aberbeeg, Abertillery, NP13 2DA – a low and medium secure service for men that aims to provide an individualised formulation of needs, ensuring care and treatment are appropriate. Aberbeeg is located within the boundaries of the county borough of Blaenau Gwent, set in 11.5 acres of land with beautiful views of the Welsh countryside. The service utilises a twelve week therapeutic planning model alongside the “Safewards” model of care, with therapeutic timetables reviewed and updated as service users progress.
