Carers Leeds wants to improve the lives of unpaid carers across the city. We provide information, advice and support to unpaid adult and parent carers in Leeds. We aim to influence others to act and create positive change for unpaid carers.
What is an unpaid carer?
An unpaid carer is someone who provides help and support to a family member, friend or neighbour who couldn’t manage without their help. This could be due to illness, disability, mental health problems or substance misuse.
What we do
Established in 1996, Carers Leeds is a charity which provides information, advice and support to unpaid adult and parent carers across our city. We have an Advice Line, carers support groups, one to one support and support in hospitals. Some of our services are open to all carers and some are targeted at specific groups of carers. Our services are provided to communities throughout Leeds.
We work in partnership with others to deliver our service and to influence them to act to benefit unpaid carers.
Brief Role Description:
This is an excitingopportunityto become part of Carers Leeds supportteam within Leeds Teaching Hospitals Trust (LTHT). We are looking for a Carer Support Worker who has a range of knowledge and skillsto providespecific support to carers who are caring for someone who is an LTHT patient, carers who are themselves patients and also to LTHT staff who may have caring responsibilities.
Weparticularlywish to encourageapplicationsfrompeople who have knowledge andexperienceof working within a hospital setting and who understand hospital processes.We are looking forsomeone whois highly motivated and creativewith good assessment and casemanagementskills to support carers, many of whom will be supporting someone with complexhealthneeds. We would want the person to workwith astrength-based/outcome focusedapproach.
Thepostholder willprovidesupport 1-1 support either face-to-face within LTHT or by telephone but will also havea good understanding of how to support people through digitalplatforms and groups.In particular the successful candidate will be expected to build strong relationships with hospital wards and key staff at Chapel Allerton Hospital to develop the service within this setting.
Key Duties and Responsibilities:
To provide person-centredinformation, advice and supporttocarers who care for someone who is an in-patient in LTHT or where the carer is an in-patient.
To manage a case load of carerswho are supporting patients with often complex physical and mental health needs and to become an expert in understanding the issues faced by carers who have contact with LTHT.
To support carers through the hospital discharge process including the provision of help to source appropriate care packages for the person with care needs.
To develop close working relationships with hospital wards and key LTHT staff across all LTHT sites but with a specific focus on Chapel Allerton Hospital.
