Job Purpose
The Team Leader is responsible for supporting the Registered Manager and Deputy Manager in the day-to-day running of the residential service. The role involves leading staff on shift, ensuring high standards of person-centred care for individuals with mental health needs, and maintaining accurate and up-to-date documentation including care plans, risk assessments, and related records.
The Team Leader will ensure that residents receive safe, effective, and compassionate support while promoting independence, wellbeing, and recovery.
Key Responsibilities
Leadership and Shift Management
* Lead and coordinate staff during shifts to ensure the smooth running of the service.
* Act as the senior member of staff on duty and provide guidance and support to care staff.
* Oversee the day-to-day delivery of care and ensure all staff follow care plans and organisational policies.
* Ensure the environment is safe, supportive, and responsive to residents’ needs.
* Address any incidents, concerns, or safeguarding issues appropriately and report to management.
Care Planning and Documentation
* Develop, update, and maintain person-centred care plans in collaboration with residents and professionals.
* Complete and review risk assessments to ensure the safety and wellbeing of residents.
* Conduct Mental Capacity Assessments where appropriate in line with legislation and best practice.
* Develop and maintain Safety Plans, Life Plans, and “All About Me” profiles to support personalised care.
* Ensure all documentation is accurate, clear, and completed in line with regulatory and organisational requirements.
* Complete monthly reviews of care plans and ensure any changes in needs are recorded and communicated to the team.
Meetings and Communication
* Attend and contribute to multi-disciplinary meetings, reviews, and internal service meetings.
* Lead and facilitate team meetings and handovers when required.
* Communicate effectively with residents, families, external professionals, and colleagues.
* Ensure relevant information is shared with staff to maintain continuity of care.
Staff Support and Supervision
* Provide regular supervision and guidance to staff members.
* Support the development and training of care staff.
* Monitor staff performance and provide constructive feedback.
* Promote a positive team culture and encourage good practice within the team.
Quality and Compliance
* Ensure care delivery complies with legislation, safeguarding procedures, and regulatory standards.
* Support audits and quality monitoring within the service.
* Ensure accurate recording of incidents, safeguarding concerns, and daily records.
* Promote best practice in mental health care and recovery-focused support.
Resident Support
* Support residents with their daily living needs in accordance with their care plans.
* Encourage independence, personal development, and wellbeing.
* Build positive relationships with residents and support them to achieve their goals.
Requirements
Essential
* Experience working within mental health or residential care services.
* Experience in a senior support worker or team leader role (or similar).
* Knowledge of care planning, risk assessments, and safeguarding procedures.
* Understanding of Mental Capacity Act and person-centred care approaches.
* Ability to lead a team and manage shifts effectively.
* Good written and verbal communication skills.
* Ability to maintain accurate documentation and records.
Desirable
* NVQ/QCF Level 3 or above in Health and Social Care.
* Experience completing Mental Capacity Assessments and safety planning.
* Experience conducting staff supervision.
* Knowledge of regulatory standards such as CQC requirements.
