North East Fife Locality
An exciting opportunity has arisen for a health care support worker to join the North East Fife locality District Nursing Team (Ladybank/Newburgh base). You must have excellent organisational skills, effective communication and interpersonal skills. Must be a team player and have a genuine interest in working in the community.
Patients are seen within their own homes and care homes in order to deliver excellent nursing care. The main focus of the team is preventing unnecessary admission, supporting earliest possible discharge from hospital and enabling patients to remain as independent as possible in their homes.
You will be required to support the District Nursing Team to deliver excellent evidence based treatment and care to the practice population of house bound patients.
You will be required to have knowledge and experience to be able to support this role and an appropriate SVQ is desirable. Our service offers regular supervision and supports training opportunities. You must have an ability to travel across the Fife-wide division if the service warrants it and to provide evening cover as required.
This post holder will be required to work within a 24 hour, 7 day Community Nursing Service. The post will include working weekends and public holidays.
Responsibilities
* Provide care to house bound patients and residents in care homes in the North East Fife locality as part of the District Nursing Team.
* Support delivery of evidence-based treatment and care in the community setting.
* Collaborate with the District Nursing Team to prevent unnecessary hospital admissions and support timely discharge.
* Maintain high standards of professionalism and patient safety; participate in supervision and training opportunities.
* Be flexible to travel across the Fife-wide division and provide evening cover as required.
Requirements
* Excellent organisational, communication and interpersonal skills; must be a team player with an interest in community work.
* Previous community experience is advantageous; phlebotomy skills are desirable.
* Adequate knowledge and experience to support the role; SVQ is desirable.
* Ability to travel across the Fife-wide division and willingness to work evenings, weekends and public holidays.
* Be prepared to become a member of the PVG Scheme prior to appointment.
