Pulse is seeking to recruit a Band 6 Occupational Therapist to join a busy community rapid response team at an NHS client based in Taunton, Somerset.
The role is available to start as soon as possible, offering full-time or part-time hours for an initial 5 months period.
The successful Occupational Therapist will be responsible for helping the client maximise function through home visits and make referrals to hospital when needed. You will be working within a community setting within an MDT team who strive to provide a high-quality service to all users.
Duties within this role will include:
1. Providing home care packages
2. Carrying out risk assessments
3. Making hospital referrals/discharge planning
4. Devising rehabilitation programmes to develop skills
5. Dealing with equipment to assist with daily living
6. Managing own caseload
Skills and experience needed:
7. Full licence and access to own vehicle
8. Wide knowledge of long-term condition management
9. HCPC registration
10. Relevant degree/qualification
