About the Role Nurse Case Manager
As a Nurse Case Manager, you will lead and oversee a caseload of adult clients requiring complex care in the community. Youll design personalised care plans, support and clinically upskill care teams, and ensure safe, high-quality, and compassionate care delivery. You will collaborate closely with CHC teams, commissioners, MDT professionals and families to deliver holistic, outcome-focused care.
