This role has a starting salary of £27,634 per annum, based on a 36-hour working week. Part times roles are paid a pro-rated salary to the FTE.
We are hiring several new Integrated Reablement Workers to join our fantastic service. Joining us as an Integrated Reablement Worker, you will be doing something that really matters. Working autonomously, you will provide support to people within their own homes, often as a lone-worker.
We have roles available in:
1. Surrey Heath (Camberley) – 2 x 36 Hours
2. North West (Spelthorne) – 2 x 36 hours
3. Guildford and Waverley – 3 x 36 hours, 1 x 18 hours

About the Team
The Reablement Team is part of the Adults Wellbeing and Health Partnerships directorate of Surrey County Council. Our organisation is currently working on a 3–5 year transformation programme, looking at our customer journey, partnership and integration.
We are a multi-disciplinary team who provide short term support for up to 6 weeks. Support is usually provided to adults in their own homes to promote independence and choice by helping them work towards their reablement goals as set out in their support plan. We aim to help improve people's quality of life, maximising their long term independence and enabling people to remain or return to live in their own home within the community.
We are an Occupational Therapy-led service and our community workforce includes Occupational Therapists, Specialist Workers, Team Leaders, Integrated Reablement Workers and Reablement Assistants. Our team uses a strengths-based approach when supporting people to achieve their goals. We work closely with some of our private providers across Surrey. They are part of our Collaborative Reablement Service (CRS) and are an extension of our Reablement Team.
About the Role
As an Integrated Reablement Worker (IRW) you will carry out initial visits to people referred into the Reablement Teams. You will work with the individual, and/or families and carers if appropriate, in setting up their support plan with reablement goals. The support plan aims to identify how the person being supported can build on their strengths to become independent with daily living tasks. This may include tasks such as personal care, medication support and meal preparation.
As an IRW you will complete initial assessments and review people's support plans to ensure that the plan is continuing to meet the needs of the person. You will work closely with our Occupational Therapists and Team Leaders, attending Multi-Disciplinary Team (MDT) meetings to review caseload.
As an IRW you will support with the induction of our new recruits and provide support to our Reablement Assistants.
IRW's will undertake the Trusted Assessor Training within 6 months of starting at Surrey. Trusted Assessor training will enable you to prescribe community equipment such as raised toilet seats and bed levers. These are usually small equipment items that can help the people we support achieve their goals and become independent with their daily living tasks.

15. Strong communication skills with service users, families, carers and professionals.
16. Competence in managing caseloads, maintaining accurate records and using IT systems.
17. The flexibility to work across different areas and build effective partnerships.
Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area.Reasonable adjustments where needed will be made for successful applicants who have a disability or long-term health condition to enable them to fulfil the requirements of the job.
