Community Mental Health Team
Location: South Wales
Pay rate: £38 per hour
We are seeking experienced and passionate Social Workers to join the Community Mental Health Team. Ideally, candidates will have 2-3 years of experience within Adults Social Care. In this role, you will uphold and champion best practices, focusing on early problem-solving to meet the needs of vulnerable adults and reduce risks. The position involves a mix of office-based work and visiting clients in hospitals and their homes. Mental health experience is essential, along with report writing, attending Mental Health Tribunals, and familiarity with WCCIS would be advantageous. Local candidates or those willing to travel are preferred.
This team collaborates with various professionals, including health board colleagues, therapists, external providers, and families. It’s an exciting time to join adult social care with supportive, experienced colleagues committed to citizen-centered service delivery.
Working within social care legislation and local government requirements, you will support the delivery of person-centered, safe, and effective social care services. Your professional knowledge and experience will enable you to make assessments and determine appropriate interventions. Using your social work expertise and communication skills, you will establish and maintain relationships with service users and agencies to meet their needs. Your commitment to positive, sustainable outcomes for adults is vital, along with a passion for supporting adults and their families to thrive. Ideal Candidate:
* Qualified Social Worker registered with Social Work England
* Experience working with adults and families, including vulnerable groups
* Creative, flexible, and forward-thinking approach
* Knowledge of Child Protection legislation and issues
* Continuous improvement mindset to achieve the best outcomes
* Excellent mediation and negotiation skills
* Skilled in building relationships with adults and families for assessments and plans
* Dedicated to delivering high-quality services
* Experience in multi-disciplinary, interagency work
* 4+ years of qualified experience
Role Responsibilities:
* Conduct comprehensive assessments of risks, protection, and social care needs
* Manage an agreed caseload
* Utilize specialist communication skills for optimal outcomes
* Create individual care plans respecting diversity and promoting dignity and empowerment
* Provide advice and support to unqualified staff and students for development
