Joiningour Clinical Pharmacy Team across General Practices and Care Homes in SurreyHeath and playing an important role complementing the clinical pharmacists andother members of the multi-disciplinary team (MDT). Working under supervision of a clinicalpharmacist, undertaking resident/patient-facing activities and supporting theteam of clinical pharmacists to ensure effective and efficient use of medicineswithin care homes and general practice, through shared decision-makingconversations with residents/patients.
Initially focusing on supporting the Practice(s) and Primary Care Network (PCN) implementthe medicines optimisation requirements of the new Network Contract Direct EnhancedServices and other Practice and PCN responsibilities. As our plans for the PharmacyTechnician role mature, the focus of the role will expand accordingly to ensure effectivesupport to the clinical pharmacy team and to meet the needs of the practice(s) and thepopulation served.
A key focus will be supporting the GPs and clinical pharmacists in implementing effectivemedicine management in care homes and across the practices population, and identifyingareas for improvement and initiating and managing change.
Main duties of the job
* Functionas part of the general practice and PCN team and working across the practicesand care homes in Surrey Heath, developing professional supportive relationships.
* Carryout medicines optimisation tasks, to improve the quality and cost effectivenessof prescribing, and patient adherence and administration.
* Undertaketasks to support structured medication reviews/medication reviews.
* Buildand run EMIS reports to identify/stratify patients/residents for review.
* Undertakemedicines reconciliation, with support of other clinical staff reviewmedication changes and communicate change appropriately, including for new carehome residents / patients and post hospital discharge / out-patients.
* Synchronisingmedicines for patient transfer between care settings, linking with localcommunity pharmacists, and referring to the clinical pharmacist for structuredmedication reviews (SMRs).
* Useconsultation skills, where required, to work in partnership withpatients/residents to ensure they use their medicines effectively (e.g.checking inhaler technique).
* Wherecompetent, provide specialist expertise to address both the public health needsand social care needs of residents/patients, including providing lifestyleadvice, service information, and helping tacking health inequalities.

Knowledge, Training, Skills and Experience
* GCSEs including English and Maths (C and above) or equivalent
* NVQ Level 3 in Pharmacy Services or equivalent
* Registration with General Pharmaceutical Council (GPhC).
* Have completed, be enrolled in, be undertaking or prepared to undertake an approved primary care pharmacy educational pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).
* Knowledge of pharmacy procedures and the wholesaler supply chain.
* Skilled in using pharmacy references, both online and texts (including the ICB's formulary).
* Understanding of medicines legislation and ethics.
* Knowledge of diversity issues and ability to work in non-discriminatory way.
* Experience in medicines management or community pharmacy
* Experience of General Practice repeat prescribing policies and systems -Use of GP computer systems, IT skills - in analysing and presenting prescribing activity data
