Job summary
An exciting opportunity has arisen for a band 7 Clinical Team manager to lead our HVU team.
The key aim of the service is to reduce the number of unplanned attendances to the acute hospital by facilitating and coordinating multidisciplinary support and intensive, time-limited interventions to patients who frequently attend A&E. High intensity users of A&E departments have been identified as having a high impact on the wait times and flow of the hospital, other emergency services and health systems.
The service will ensure the coordination of care and the provision of an assertive outreach approach to vulnerable groups of patients in the community, who typically disengage with mainstream services and / or have a disproportionate use of emergency services (acute hospital and ambulance services).
We work with other health and social care professionals which may include GP, MH services, social care and many more ensure the patients are accessing appropriate services to meet their needs and to formulate a single personalised care and risk management plan.
You will be responsible for a small case and to provide day-to-day oversight of operational and clinical management. We are looking for someone who will be enthusiastic, self-motivated and have excellent communication skills. The post holder will work closely with the UHNM and must be able to work collaboratively with all system partners.
The team operates Monday-Friday, 09.00-17.00 and are predominately home based.
Key Duties/Responsibilities
* Management and clinical responsibility for the Team, ensuring processes are in place for the delivery of safe, compassionate care in accordance with Trust policies and procedures
* To provide strong clinical leadership as well as work as a member of the multi-disciplinary team.
* Lead a multi-disciplinary team of staff in the delivery of high-quality evidence-based practice, ensuring that regular supervision and appraisal takes place.
* Ensure systems are in place to manage administrative and operational resources in a cost-effective manner, evaluating the standard of service delivered.
* Co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, to ensure effective interface and continuity of care between disciplines and services.
* Role model best practice, where appropriate undertaking clinical procedures to a standard that ensures safe and effective care and complies with the relevant Code of Conduct or relevant professional bodies.
* Management of clinical care within the team, ensuring that clinical pathways and Trust values underpin the practice and delivery of care on the unit.
* To work within and across professional and organisational boundaries to deliver integrated, individualised and expert care and leading communication with all stakeholders.
* The post holder will work alongside the Service Manager. To support with the day-to-day management, quality and performance of the service.
* Demonstrate highly developed clinical skills and act as clinical advisor and educator to junior staff.
* To provide support and guidance to colleagues over issues of risks and / or complex clinical presentation.
Person Specification
Qualifications
* A registered mental health practitioner with a degree in nursing, social work or occupational therapy.
Experience
* Post qualification of work in core profession.
