We are looking for a motivated and organised Administrator to join the Transfer of Care Hub, supporting the coordination of adult discharges from hospital into community services. We are a relatively new service and our expansion has led to us needing vital administrative support. This is a busy, team-based role working closely with ward staff, community teams and partner organisations.
The post holder will provide key administrative support by maintaining accurate records on SystmOne, Excel spreadsheets and the Transfer of Care Dashboard, ensuring information is kept up to date to support safe and timely transfers of care. The role involves regular liaison with professionals to share updates and support effective communication across services.
Responsibilities include:
* Act as a point of contact for the Transfer of Care Hub, responding to routine queries from wards, community services and partner organisations.
* Update and maintain patient and service information on SystmOne, Excel and the Transfer of Care Dashboard.
* Track patient progress through discharge pathways and update systems in line with agreed processes.
* Liaise regularly with ward teams, therapists and discharge services to support timely transfers of care.
* Answer telephone calls, manage messages and pass information to the appropriate staff.
* Support the organisation of meetings, including diary coordination and circulation of information.
* Maintain organised electronic and paper filing systems.
* Provide general administrative support to the Hub team as required.
* Ensure information recorded on systems and spreadsheets is accurate, timely and complete.
* Support effective communication between hospital and community services to reduce delays in discharge.
* Adhere to Trust policies on confidentiality, information governance and data protection at all times.
* Support service reporting by collating data and meeting required submission timescales.
* Raise data quality, system or process issues with senior colleagues when identified.
* Maintain a professional and helpful approach when dealing with all internal and external contacts.
* Participate in training and keep knowledge up to date with discharge processes and local procedures.
* Provide flexible administrative support as the service develops, in line with role and competence.
