Job Overview
Exciting opportunity for a Case Coordinator in the West Integrated Transfer of Care Hub (iToCH), based in Helston Community Hospital. Expected to hot desk and shadow staff during induction at Camborne Health Office and Penwith Health Centre. Key role in supporting clinical staff with admission avoidance, arranging community support, case managing non‑urgent referrals and facilitating safe and timely discharge from acute and community hospitals.
Main Duties
* Achieve a collaborative approach to progression of community referrals and, at times, hospital discharges, ensuring timely quality care.
* Coordinate patient needs, utilising all available resources and capacity.
* Obtain information from health and social care professionals, patients and families to track progress and ensure quality care delivery.
* Maintain communications with the team or referrer regarding timing of interventions, equipment ordering/delivery and organisation of ongoing care.
* Liaise with other agencies and departments to provide efficient and effective communication for care delivery.
* Develop working relationships with community health and social care teams to enable support across boundaries.
Person Specification – Education & Qualifications
* Level 4 qualification in administration, health or social care, or equivalent knowledge or experience.
* Sound administrative experience.
* Knowledge of Microsoft Office Applications.
