The post holder will work to support the role of the Health Visitor in ensuring the public health programme is carried out within the designated pre-school population. Based within a community setting, the Nursery Nurse will provide a service to families as identified in the needs assessment carried out by the Health Visitor. He/she will take a key role in delivering parent support, health promotion and programmes of care. The post holder will be a team player and contribute to the overall health care offered to individuals, families and communities.
Main duties of the job
* Work with a multi-agency Flying Start team to achieve the Welsh Assembly Government initiative scheme, targeting communities in need to provide enhanced local services within a multi-agency setting.
* Facilitate one-to-one, group and community work to address health inequalities in partnership with the wider multi-skilled team.
* Deliver parent support, health promotion and programmes of care; provide a service to families as identified in the needs assessment carried out by the Health Visitor.
* Undertake planned packages of support through implementing and evaluating planned programmes of care.
* Contribute to the public health role in line with government public health initiatives.
* Take part in induction, clinical supervision and safeguarding support as provided by the Health Board; participate in in-house training where available.
* Welsh language skills are desirable; applications from Welsh and/or English speakers are welcome.
