* To manage and maintain the team's clinical caseload, including all associated administration tasks.

* To use discretion, sensitivity and deal with information in a confidential and professional manner at all times.
* Oversee and monitor the distribution/collection of equipment that is used to monitor patients in their own home (tech-enabled care).

* To be responsible for data collection, based on the outcomes from the clinical caseload.
* To participate in and organise team audits when appropriate.

The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trust's policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately.

In order to be considered, the role requires a standard DBS check at a cost of £26.90, which will be deducted from your first Trust salary. The successful candidate will also join the DBS update service and pay the annual subscription fee of £16.
