Were looking for an enthusiastic Primary Care Clinical Pharmacist (Independent Prescriber) to join our friendly GP practice in East Kennet. Working part time (30 hours per week), youll be part of a close-knit practice team where your clinical expertise truly matters. Youll provide trusted pharmaceutical care, resolve medication queries, and deliver safe, effective advice that helps patients live healthier lives.
What youll be doing
* Acting within your professional boundaries as part of a multidisciplinary team
* Carrying out structured medication reviews (including SMRs and polypharmacy reviews)
* Providing expert advice to both patients and clinicians
* Supporting the practice team with prescription and medication queries
* Conducting medicine reconciliations and audits to drive service improvements
* Leading on medicines optimisation and promoting quality use of medicines
* Helping improve patient outcomes and access to healthcare

What were looking for
* GPhC-registered Clinical Pharmacist with an Independent Prescribing qualification
* 612+ months experience in a Primary Care or GP Practice setting
* Completion of the CPPE Primary Care Pathway (or exemption)
* Experience using SystemOne
* Excellent communication and interpersonal skills
* Able to commute to or live near East Kennet
