Overview
This is an exciting opportunity for experienced Registered Mental Health Nurses looking to join the community mental health services in the central area. You will have excellent communication skills, be self-motivated, proactive and committed to the promotion of innovation for service delivery. The successful candidate will deliver holistic, safe, effective care to patients within the community and hold a complex caseload of individuals aged 18-65 with severe and enduring mental health issues. You will work collaboratively to assess needs and create care and treatment plans to meet identified needs and risks. The post holder will work independently and liaise with a variety of agencies and third sector organisations, plus the individual’s carers and family members. This post will be located within the Community Mental Health Team in Denbighshire with potential to work within other teams and, in the future, possibly in inpatient settings. You will have exceptional communication skills and the ability to build and maintain effective working relationships, demonstrating a “can do” attitude.
Main duties of the job
* Undertake Care Coordination of service users with complex mental health needs and promote and maintain a high standard of community psychiatric care, both independently and with others from the MDT. This may involve being part of a clinic‑led system for some service users.
* Work in a recovery‑focussed way and facilitate user and carer involvement.
* Desirable to have Welsh language skills; Welsh and/or English speakers are welcome to apply.

Qualifications
* Registered Mental Health Nurse, educated to Diploma level or equivalent
* Evidence of Continued Professional Development
Experience
* Essential: Experience in Mental Health Services for older people
Desirable criteria
* Broad range of experience working in health and social care settings
Knowledge
* Essential: Demonstrate a good understanding of mental health problems in the older person
Desirable criteria
* Awareness of current developments within MHLD older peoples service
